PATIENT INFORMATION

Hours of Operation

  • Practice hours: Monday - Thursday 8:00 am- 4:00 pm , Friday 8:00 am - 12:00 pm , Weekends Closed 

  • The office will be closed to observe major holidays.

New Patients:

  • If you are new to our practice you must first complete a "New Patient Packet." 

  • After we receive your forms, our staff will call you to schedule and appointment. Please allow 1 business day for our staff to process your forms. 

  • After the appointment is made, you will receive an email with an invitation to create an account with Breeze. Breeze is our patient portal where you can communicate with your provider.  

  • Initial evaluation appointments may last up to 60 minutes.

  • If the patient is a minor (under 18), both the child and a parent/guardian must be present for the duration of the initial evaluation.

  • Please request records from prior psychological evaluations including your most recent psychiatric visit note and have them faxed to our office. 

  • Please e-check in 5-10 minutes prior to your scheduled initial appointment through Doxy. On the morning of your appointment, we will text you a link to access your virtual appointment. You can also access your virtual appointment link on our website, located in the top right corner of the homepage.. 

  • Please note that if you log on to Doxy late for your appointment, you may be asked to reschedule. If you cannot be reached by your provider within 9 minutes of the start time of your appointment it is considered a no-show and you will have to reschedule and be charged a fee.

  • No show or less than 24 hr cancellation of your new patient appointment is subject to a $300 fee.  This fee is patient responsibility and not covered by insurance.

  • All patients are required to have a Breeze account. This is our patient portal. Through Breeze, you will be able to make, cancel, or reschedule appointments; update demographic information; view your account balance; and message your provider.

Established Patients:

  • You may cancel and reschedule your own appointments through the patient portal "Breeze" 

  • Please e-check with Doxy in at least 5 minutes prior to your scheduled appointment, you will be added to the virtual waiting room. Your provider will start your appointment at the scheduled time.

  • If you need to update any demographic (address, phone number, etc.) information, you can do so on Breeze. To update your insurance please complete an "Insurance submission form" 

  • Follow-up appointments are scheduled for 20 minutes or 40 minutes depending on how much time the provider needs with each patient.

  • Should you need to cancel/reschedule your appointment, please do so at least 24 business hours prior. Missed appointments and late cancellations are subject to charges ($75 for 20 min follow-up and $150 for 40 min follow-up). 

  • Please note that if you log on to Doxy late for your appointment, you may be asked to reschedule. If you cannot be reached by your provider within 9 mins of the start time of your appointment it is considered a no-show and you will have to reschedule and be charged a fee. 

  • If the patient is a minor (under 18), both the child and a parent/guardian must be present for the duration of all appointments.

  • All patients will be required to have standard labs on file annually as well as a physical or wellness visit by their Primary care provider at minimum. If you are prescribed a controlled substance this will include a urine drug screen.

  • Allow 48-72 business hours for return messages.

Refills:

  • Patients are required to be seen every at least every three months in order to receive medication refills.

  • Please contact your pharmacy 3–5 days before running out of medication. The pharmacy will submit a refill request to our office on your behalf.

  • Kindly allow 48–72 business hours for processing refill requests.

Raleigh Wellness and Behavioral Health does not have after-hours answering services and does not provide emergency care. If you feel that you or someone you know may be experiencing a crisis or emergency, please call 9-1-1 or go to the nearest emergency department for evaluation.
