Getting started is simple.

Whether you are a new patient or established, here you will find the information you need to make appointments

New Patient 

If you are a new patient to our practice please start by completing a new patient packet. Once completed, we will call you to schedule your new patient appointment within 1 business day.

The "New Patient Packet" consists of 3 separate sections.

Once you complete section 1, you will be directed to section 2, and then section 3. Should you close your browser after section 1, you may use the links above to get to the others.

  1. New Patient Information

  2. Insurance Card (picture submission)

  3. New Patient Assessments

 

 

To complete this form you will need:

  • Insurance card

  • Medication list

  • Allow 20-25 minutes to complete all 3 sections

Is Telehealth right for me?

Video call

Many patients face barriers when it comes to in-person healthcare visits. Whether due to disabilities, physical distance, or transportation, many patients are able to receive medical consultations via telehealth they may otherwise have missed.

​

Much like an in-office visit, your provider will review the medical chart and history, and then speak with you. Connecting with your provider is easy, you may conduct your visit from a smartphone or computer with a static link that is assigned to each provider.

 

And, yes, medications can be prescribed via tele health!

 

Telemedicine

All appointments are virtual. You can have your initial and subsequent appointments from the comfort of your home or office.
